Zeltiq Aesthetics is accepting applications for its CoolSculpting Shape of Summer Sweepstakes. The contest is including the U.S and the District of Columbia citizens who are 18 years of age or above.
Zeltiq Aesthetics is a subsidiary of Allergan based in Pleasanton, California formed in 2005. It works to market and licenses devices used for the cryolipolysis procedures.
Who May Eligible?
To be eligible, the applicants must be following the entire eligibility criteria:
- Must be legal residents of the fifty (50) United States and the District of Columbia
- Must be at least eighteen (18) years old at the time of entry
- Licensed health care providers, customers, employees and contractors of ZELTIQ Aesthetics, Inc., an Allergan affiliate, Allergan, Merkle, Inc., and their parent and affiliate companies as well as the immediate family and household members of each such employee are void.
How to Apply
Participants can fill out their information electronically by filling out an online entry form to be applicable for the contest. The form is displayed on the bottom of the main page of the sweepstakes.

Benefits
The CoolSculpting Shape of Summer Sweepstakes will be awarding one Grand Prize of $25,000 check and also five prizes of $2,500 check.
