- A copy of the most current income tax return or
- W-2 form Copy of either social security.
- Disability check Copy of your last four (4) consecutive pay stubs Copy of your most current bank statement showing monthly deposits of income.
- If Candidates is unable to provide any of the above documents,
- A letter from a caseworker or a letter of explanation from you regarding the financial situation needs to be attached.
